Project Goals

  • Develop a framework to identify, pilot, and evaluate CHW and community resource financing models in two different regions of Virginia ​

  • Create conditions for healthcare organizations to build partnerships with CBOs and other organizations to increase their capacity, address health disparities, and lower costs for all​

QV Health Solutions Role

  • Identify partner organizations whose outcomes align with those of state MCOs​

  • Conduct interviews to identify and establish partnerships with organizations whose outcomes align with those of MCOs and other payers in the state

  • Provide education and training to government, MCOs, and other key stakeholders on the outcome-based financing model​

  • Assess the willingness and ability of stakeholders to implement an outcomes-based financing model. ​

  • Support deal development, execution, and implementation of outcomes-based financing contract​

  • Develop outcomes-based financing and implementation plan for selected programs and regions​

Funded through a national grant from the Center for Disease Control and Prevention, IPHI deployed CHWs as a strategic intervention to assist communities in response and recovery from COVID-19. The project will also develop and test innovative financing strategies to support the long-term sustainability of CHWs in specific communities. ​
